·          İthalatçı firmanın ünvanı, adresi, telefon ve faks numarası

(Name, address, telephone and fax of the importer company)

·          Nihai kullanıcının ünvanı, adresi, telefon ve faks numarası

(Name, address, telephone and fax of the end user)

Son kullanıcı olarak, ithal edilen bu kimyasalın yalnızca Kimyasal Silahlar Sözleşmesi tahtında yasaklanmayan amaçlarla kullanılacağını ve KSS’ne taraf olmayan ülkelere yeniden transfer edilmeyeceğini taahhüt ederiz.
